Job Description:
A company is looking for a Care Manager (RN).
Key Responsibilities
Develops and facilitates complex care management activities for
members with physical needs
Coordinates care between members, families, and care providers to
ensure adequate services
Monitors member status and updates care plans as necessary to meet
individual goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
Multi-state RN license in NC, SC, GA, or FL
Must reside in the EST time zone
Experience working in a remote setting is preferred
