Clinical Liaison Community Home Health and Hospice
Location: St. Jude Travel Area
Work Schedule: Full-Time, 80 Bi-weekly Hours
Shift: 8-hour, Day
The Clinical Liaison LVN is responsible for informing patients
and/or clients of available services. The Admissions Coordinator
LVN ensures that patients receive responsive and safe transition;
coordination and continuum of care after patients are referred from
hospital/Skilled Nursing Facility (SNF) to home.
