8:00 AM – 5:00 PM Job Description: Seeking an experienced
Registered Nurse (RN) to join our dedicated home health team in
Anaheim, CA. The RN will deliver high-quality, patient-centered
care to individuals in their homes across the assigned territory.
The ideal candidate is compassionate, detail-oriented, and skilled
in Oasis documentation and home health case management. Roles and
Responsibilities: Provide skilled nursing care to patients in their
homes, ensuring individualized treatment plans are implemented and
followed. Conduct comprehensive patient assessments, including
Start of Care (SOC), Resumptions of Care (ROC), Recertifications,
and Discharges (each worth two points). Complete routine follow-up
and other skilled nursing visits (each worth one point) to meet
productivity expectations of 6 points daily . Collaborate with
physicians, therapists, and other healthcare professionals to
coordinate patient care and ensure continuity of services. Educate
patients and caregivers on disease processes, medications, and home
health care procedures. Accurately document all patient
interactions and updates using the HCHB (Homecare Homebase) EMR
system . Adhere to all clinical and regulatory policies and
procedures in compliance with federal, state, and agency standards.
Demonstrate effective communication and time management skills
while managing multiple patients and travel routes. Education and
Qualifications: Required: Active and unrestricted Registered Nurse
(RN) license in the state of California. Experience: Minimum of 1
year of recent Home Health RN experience required. OASIS E1
experience required. EMR: Proficiency with HCHB (Homecare Homebase)
required. Valid driver’s license, auto insurance, and reliable
transportation. Current BLS (Basic Life Support) certification from
the American Heart Association.
